Xổ số miền bắc thứ bảy tuần trướcWelcome to the Brookdale Ambulatory Care Services (ACS), Brookdale Family Care Centers (BFCC) & Brookdale Urgent Care Center (UCC) where your care is our number one priority.  We offer high quality, compassionate health care for people of all ages, through all stages of life.  Brookdale’s Ambulatory Care Department offers Adult and Pediatric Primary Care and a variety of multi-subspecialty services to the communities we serve.  As a Patient Centered Medical Home recognized site by the National Committee on Quality Assurance (NCQA) our goal is to promote prevention and wellness. If you have a special health concern or condition, your health care team will help connect you with other health professionals to get you the care you need.

• A Personalized Primary Care Team that works together to manage ALL of your healthcare concerns in a courteous and confidential manner during your BFCC visit;
• Urgent Care, Routine and, Same-Day appointments during office hours;
• Clinical advice via phone, My Brookdale - Patient Portal () during and after office hours;
• Emergency Department (open 24 hours);
• Our answering service will relay ALL after-hours calls to our on call physician.
• You and your family are the most important part of your health. Your care is delivered with respect for your cultural background, and in your preferred language

 

The Brookdale Family Care Centers accept most major insurances.  All co-pays are due and payable at the time of service.  Please keep us informed as to any changes in your insurance so we may bill properly.

 

We also provide financial aid services for qualifying individuals based on income.  Please contact our financial counselor for free and confidential assistance, available at the main Brookdale Hospital site within the Snapper Building 4th Floor at One Brookdale Plaza, Brooklyn, NY 11212.
